Under general supervision, guided by the policies and procedures of the Medical Center and Ambulatory Services, and in collaboration with providers and administrative management, the Ambulatory GI (Gastroenterology) Nurse Care Coordinator works autonomously, utilizing the nursing process, to assess, plan, coordinate, implement and evaluate patient care within the ambulatory setting. S/he will utilize a high level of clinical knowledge to fulfill patient needs, ensure efficient patient care, and provide patient education when necessary. The Clinical Nurse II will act as a patient advocate and as a liaison between insurance companies, inpatient/outpatient services, patients/families and all other necessary healthcare services to manage and facilitate efficient and timely care received in clinic as well as coordination of care. In addition, the Clinical Nurse II will assist in the development of patient care standards and practices. The GI Clinical Nurse II is responsible for: Clinic procedures needed by GI patients including but not limited to G-Tube exchange, NG placement and management, central line care and dressing changes including DC of lines, coordinating outpatient services and needs for a panel of pediatric GI patients and providers in the ambulatory setting including assisting with insurance authorizations, contacting home health companies, assuring DME and other home supplies are adequate, assisting with medication refill requests, patient teaching and assisting in the overall coordination of timely patient care. This position requires flexibility to orient and work at all UCSF Medical Center locations.
