About The Position

Under general supervision, guided by the policies and procedures of the Medical Center and Ambulatory Services, and in collaboration with providers and administrative management, the Ambulatory GI (Gastroenterology) Nurse Care Coordinator works autonomously, utilizing the nursing process, to assess, plan, coordinate, implement and evaluate patient care within the ambulatory setting. S/he will utilize a high level of clinical knowledge to fulfill patient needs, ensure efficient patient care, and provide patient education when necessary. The Clinical Nurse II will act as a patient advocate and as a liaison between insurance companies, inpatient/outpatient services, patients/families and all other necessary healthcare services to manage and facilitate efficient and timely care received in clinic as well as coordination of care. In addition, the Clinical Nurse II will assist in the development of patient care standards and practices. The GI Clinical Nurse II is responsible for: Clinic procedures needed by GI patients including but not limited to G-Tube exchange, NG placement and management, central line care and dressing changes including DC of lines, coordinating outpatient services and needs for a panel of pediatric GI patients and providers in the ambulatory setting including assisting with insurance authorizations, contacting home health companies, assuring DME and other home supplies are adequate, assisting with medication refill requests, patient teaching and assisting in the overall coordination of timely patient care. This position requires flexibility to orient and work at all UCSF Medical Center locations.

Requirements

  • Current licensure as a Registered Nurse as defined by the California Board of Registered Nursing.
  • Bachelor’s of Science degree in Nursing (BSN) or Master’s Degree in Nursing (MSN)
  • Active American Heart Association CPR Certification
  • Knowledge and ability essential to the successful performance of the duties assigned to the position.
  • Recent experience in the use of electronic medical record documentation
  • Minimum of one year full-time RN experience in pediatric inpatient setting
  • Minimum one-year pediatric clinical experience providing nursing care for patients with diseases of the digestive system or related conditions(s) in an inpatient and/or outpatient setting
  • Recent experience in pediatric outpatient care coordination
  • Experience with maintenance of G tubes, central lines and drawing labs required.
  • Experience with insertion and maintenance of NG tubes required
  • Phlebotomy experience required
  • Experience and fluency in working with computers
  • Current Registered Nurse as defined by California Board of Registered Nursing
  • Current CPR through the American Heart Association
  • Current PALS
