About The Position

Nurse Aid (NA)-Horizon Assisted Living Part Time Night Shift- 6p-6a POSITION DESCRIPTION: Nurse Aid (NA) provides services to residents in accordance with care plans, facility policies, and procedures and at the direction of supervisor(s). Maintains established Horizon policies and procedures, objectives, performance improvement program, safety, environmental and infection control standards, and protocols. Maintains a clean, safe, comfortable, and therapeutic environment for residents and families per Horizon standards. Provides general aspects of care that meet psychosocial or physical needs or both simultaneously, within the nursing assistant’s scope of practice. Meets communication needs of residents. Treats residents and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and family. Demonstrates performance reflecting the initiative and responsibility expected of a nursing assistant. Helps in admission, transfer, and discharge of residents. Able to communicate effectively in English, both verbally and in writing. Maintains confidentiality of protected health information, including verbal, written, and electronic communications. Help with orientation of new employees. Use proper lifting and body mechanics while delivering care to residents and meet the physical demands of the position. Reports changes in a resident’s condition to nurse and/or supervisor. Maintains a working knowledge of all equipment used in treatment of residents. Maintains and replaces supplies on unit assigned. As a condition of employment, completes all assigned training and skills competency. Performs other duties as assigned, within the Nursing Assistant’s scope of practice. Physically demanding, high-stress environment Exposure to blood and body fluids, communicable diseases, chemicals, radiation, and repetitive motions Standing and walking for extensive periods of time Ability to help in evacuation of residents during emergency situations. Ability to bend, stoop, kneel, crouch, perform overhead lifting and perform other common physical movements as needed for the position. Dexterity of hands and fingers to perform patient care. Ability to lift, move, push, or pull a minimum of thirty-five pounds. May be subject to falls, burns from equipment, and/or odors throughout the day, encounter reactions from dust, tobacco smoke, disinfectants, and other air contaminants. Subject to exposure to infectious waste, diseases and/or conditions which include AIDS, Coronavirus, Hepatitis B, and Tuberculosis. Must be able to move intermittently throughout the workday. Working irregular hours including after hours

Requirements

  • Ability to read, write, speak, and understand the English language.
  • Must be a supportive team member, contribute to and be an example of teamwork.
  • Ability to make independent decisions when circumstances call for such action.
  • Ability to deal tactfully with personnel, patients, family members, visitors, government agencies/personnel, and the public.
  • Must be able to relay information concerning a patient’s condition.
  • Must have leadership, supervisory ability, and willingness to work harmoniously with and supervise other personnel.
  • Must be able to follow oral and written instructions.
  • Communicates with medical and nursing staff, and other departments.
  • Basic computer skills, including ability to navigate electronic medical record systems.
  • In-depth knowledge of long-term care practices and protocols.
  • Must be able to pass a background check and drug screening.
  • High school diploma or equivalent.
  • Registered as an Unlicensed Assistant Personnel (UAP) or completion within three (3) months of hire.
  • Basic Life Support (BLS) certification required or willing to obtain.

Nice To Haves

  • 1-year prior experience in SNF/Assisted Living preferred but not needed.
