About the position

We are looking for a Nurse Admissions Specialist to join our Farnum team! The individual in this key position will provide coordination and support to prospective clients and referral sources to include initial point of contact between clients, insurance agencies, referral sources and the Agency. The Nurse Admissions Specialist will collaborate with Admissions Team and Provider to review clients with chronic or acute medical concerns to determine Farnum ability to meet needs during treatment and clients to fully participate in program activities, complete Client Intake Process to include non-invasive visual inspection and all required Intake tasks and ensure familiarity with all Intake forms to ensure client understanding of rights and responsibilities during treatment.

Responsibilities

  • Identify medical concerns during Client Intake Process. Use nursing judgement to determine if client should be sent for additional medical clearance. Alert Provider of health issues identified during Intake Process.
  • Alert Provider of need for client post admission H&P
  • Arrange for Medical Clearance with Medical Director, 24-48 hours prior to client arrival.
  • Review/Audit admissions occurring after Admission Specialist work hours to ensure all aspects of Client Intake Process are complete. Report gap trends to Supervisor.
  • Enter client medications into Aura MAR.
  • Collaborate with MNA and Provider team to ensure no gap in medication dosage
  • Collaborate with MNA Team for PA approval for medications
  • Upload incoming client documentation into EMR, alert Provider for review.
  • Obtain needed medical, clinical records and treatment as assigned.
  • Perform duties, as requested by Director, of SUT RN or LPN on Detox Unit
  • Answer and respond to assigned inquiries from potential clients, families, referral sources and organizations seeking information about the Agency's substance use treatment program.
  • Utilize RN or LPN background to complete the admissions counseling process which includes social, educational, funding information and a thorough medical assessment to determine appropriate placement for client with regards to specific program and level of care within the program.

Requirements

  • Licensed with the State of New Hampshire as a Registered Nurse (RN) or Licensed Practical Nurse (LPN)
  • Associate's degree preferred
  • Minimum of two (2) years' experience in the area of addiction services preferred
  • Valid driver's license and reliable transportation required
