About The Position

Performs advanced (senior-level) nursing work. Work involves planning, developing, coordinating, and evaluating nursing activities and providing for the care and treatment of IDD consumers under direction of the HCS Program Administrator. Work may include serving as a resource on nursing practices by performing consultative and administrative work. Works under limited supervision with considerable latitude for the use of initiative and independent judgment. Must possess strong computer skills and be able to assist with training direct care staff on delegated duties in the Electronic Health Record. This position follows a hybrid model; with work being completed in an office setting, the community and work from home allowed, as needed.

Requirements

  • Experience in nursing work.
  • Graduation from an accredited four-year college or university with major course work in nursing or from an accredited nursing program is generally preferred.
  • Must be licensed as a Registered Nurse in the State of Texas or a state that recognized reciprocity through the Nurse Licensure Compact.
  • Knowledge of nursing techniques and procedures; of health care laws and regulations; of medical diagnoses and procedures; of accepted medical treatment patterns; of community health and nursing care principles, practices, and procedures; of program regulations and procedures; and, of utilization review and control measures.
  • Ability to provide patient care; to prepare and maintain records; to explain public health law; to organize, coordinate and evaluate nursing activities and delivery of public health services; to interpret policies and procedures; to interpret health care regulations; to recognize patterns of medical necessity treatment; to communicate effectively; and, to supervise the work of others.
  • Ability to proficiently utilize computer programs, e.g., Microsoft Word and Excel

Responsibilities

  • Services are to be provided in any environment in which the client needs services/supports. While this list is not inclusive, examples include: clients home (individual, family home, group home, host home), community settings, medical offices/hospitals, Day Habilitation/Individualized Skills and Socialization (ISS) settings.
  • Responsible for the ongoing implementation and monitoring of nursing protocols per the HCS Nursing Guidelines and Local IDD Authority Service Contract for General Revenue and Crisis Respite Services.
  • Administers or oversees/delegates the administration of medication and treatment for clients, and observes consumers for unusual symptoms and reactions. Provides medication training as appropriate.
  • In conjunction with the HCS Program Administrator, evaluates reviews nursing activities and makes recommendations for improvements for quality assurance purposes. This will include review of billing and productivity monthly.
  • Conducts comprehensive nursing assessments per HCS guidelines and on approved HHSC forms.
  • Develops nursing in-service training and educational programs. This will include training on how to complete electronic Medication Administration Record (eMAR) in the Electronic Health Record. Works closely with IDD Quality Assurance and HCS Program Administrator to maintain training records.
  • Triages referrals for IDD Crisis Respite and General Revenue nursing request. Completes activities or coordinates completion of required nursing activities for all programs.
  • Instructs, counsels and assists patients in meeting their health care needs. Monitors and reviews nursing services on the consumer's IPC.
  • Provides consultation, training and technical assistance to agency staff, care recipients, provider agencies, and the medical community.
  • Process and review reports that assist in program needs, such as the daily Triage Report from Nursing Innovations, eMar report in MyAvatar, etc. Utilize reports to improve processes. Process monthly cycle and new medications. Ensure medications are distributed in a timely matter.
  • Serves as a resource on nursing practices and education programs by performing consultative and administrative work.
  • Will serve as a secondary staff in scheduling/monitoring medical appointments and completing Physician Consultation Forms for medical appointments. Must stay compliant with all required training.
  • Performs related work as assigned. Demonstrate high ethical standards, trustworthiness, and personal integrity.
  • Regular attendance, dependability, and promptness are required for the 9-3 work day or agreed upon hours 100% of the time, to ensure consistency and completeness of program’s processes.
  • Professional Development - Successfully complete training required to maintain skill competency and cross training to ensure consistency across program procedures. Complete continuing education as required by professional standards and the Center.
  • Frequent daily driving within the catchment area is required.
