About The Position

PATIENT CARE MANAGEMENT Assist Physicians in providing care and continuity of service to patients within his or her specialty by: 1. Performing a health status assessment of the patient which includes a health history, a physical examination, and a systematic recording of the data. 2. Initiating and/or performing selected procedures and laboratory examinations for diagnostic screening and referral purposes. 3. Discriminating between normal and abnormal findings and making appropriate referrals to the physician for evaluation and/or supervision. 4. Initiating appropriate treatment modalities in prescribing medications, assistive devices and other equipment. 5. Teaching, counseling and assisting the patient and family in meeting health needs, making referrals to other health professionals and agencies as required. 6. Participates in diagnostic testing to the extend allowed within their scope of practice. TEACHING AND STAFF DEVELOPMENT 1. Acts as a consultant to nursing staff and when appropriate participates in inservice training for the staff and in the community if needed. 2. Assists in clinical orientation of students or staff to the routines and policies utilized in patient management. 3. Selects and provides resource materials for nursing staff. III. RESEARCH 1. Participates in research projects within the medical specialty.

Requirements

  • NYS licensed/registered NP or PA required (NYS NP specialty area of certification must be consistent with practice).
  • Must have NPI number and be Medicare Enrolled with Medicare Identification Number (or PTAN) OR be eligible for Medicare Enrollment within 6 months of hire (eligibility criteria for NP requires National Certification through AANP or ANCC, and Master's Degree) (eligibility criteria for PA requires graduation from an accredited PA program and National Certification through NCCPA).
  • Initial certification must be present or obtained within 6 months of hire.
  • Current BLS certification through the American Heart Association or the American Red Cross is required.
