About The Position

Join a compassionate and collaborative Palliative Care team dedicated to providing patient-centered care. Our team of 16 fosters a supportive environment where colleagues work together to ensure the highest quality of care for patients and families navigating serious illness. As a NP/PA on our Palliative Care team, you will play a vital role in improving the quality of life for patients with complex, chronic, or life limiting conditions. You'll work closely with physicians, nurses, social workers, chaplains and other specialists to deliver comprehensive care that addresses physical and emotional needs. This is a full time, weekday position with hours from 8:00am-4:30pm. This position is involved in a team-based approach to care. Team members are trained to meet the highest level of function for their role as per the State of Tennessee/Georgia guidelines. Is trained and assigned (as per their role and responsibilities) to coordinate care for patients and is trained and assigned to support patients / families/caregivers in self-management, self-efficacy and behavior change. Is trained and assigned (as per their role and responsibilities) to manage the practices patient population. Participates in the practice's quality improvement process and performance evaluation.

Requirements

  • RN with a Master's Degree in nursing certification in family and/or adult nurse practitioner.
  • Graduate of a nationally accredited school of nursing.
  • Minimum of 2 years' experience in Hospice or Palliative care.
  • Current licensure to practice nursing in the State of Tennessee.
  • BLS completed prior to going to the unit/department and to be maintained going forward.

Responsibilities

  • Develop evidence-based protocols and utilize them to create individualized plans of care.
  • Collect, document, and analyze patient data.
  • Collaborate with the healthcare team in the development and implementation of testing and treatment according to protocols.
  • Participate in the development and implementation of health screening programs for adults.
  • Develop protocols for the identification of high-risk individuals.
  • Demonstrate knowledge of growth and development principles across the lifespan.
  • Assess data reflective of the patient's status and interpret appropriate information to identify each patient's requirements relative to age-specific needs.
