This is a part-time position to work weekends and one weekday to be determined by candidates preference.

Requirements

• NYS licensed/registered NP or PA required
• NYS NP specialty area of certification must be consistent with practice
• Must have NPI number and be Medicare Enrolled with Medicare Identification Number (or PTAN) OR be eligible for Medicare Enrollment within 6 months of hire
• Eligibility criteria for NP requires National Certification through AANP or ANCC, and Master's Degree
• Eligibility criteria for PA requires graduation from an accredited PA program and National Certification through NCCPA
• Initial certification must be present or obtained within 6 months of hire
• Current CPR certification through the American Heart Association or the American Red Cross is required

Responsibilities

• Collaboration with the Attending Physician and a multidisciplinary team
• Provide comprehensive health assessments and develop a plan of care for a diverse and complex patient population
• Support a robust ambulatory clinic including infusion services
• Document in the medical record by recording subjective, objective data, assessment and plan
• Record data in a consistent, legible, accurate, concise and timely manner
• Provide patient/family education
• Utilize prescriptive authority working in collaboration with the attending physician
