About The Position

Eldercare Fargo, ND (Osgood) We are seeking an NP/PA to join the Eldercare team in providing exceptional healthcare to our adult patient population. This professional, experienced and collegial team currently consists of two (2) physicians, three (3) advanced practice clinicians and RN Program Manager. Step into a busy and growing practice that allows you to be an engaged and appreciated member of an outstanding team-based model of care. 1.0 FTE, scheduled Monday – Friday during business hours Office based at Osgood Campus; will provide services to area nursing homes and assisted living facilities Eldercare call coverage (24/7) is via phone and scheduled 1:5 (1 day/week – every 5th weekend) Patient encounters per day will vary The Nurse Practitioner or Physician Assistant utilizes advanced health assessment, knowledge, and decision-making skills, while working both independently and in collaboration with physicians and other health care professionals. The NP/PA will provide health care to individuals and families, emphasizing health promotion and disease prevention. They may care for patients as noted in age-related category and area of expertise. Position requires high level of customer service skills to establish and enhance positive relationships with patients, co-workers, and others.

Requirements

  • Licensure/DEA in ND & MN required
  • Master’s or Doctorate degree in Nurse Practitioner or Bachelor’s/Master’s degree in Physician Assistant program from an accredited program/institution.
  • Current licensure as a registered nurse in appropriate state (if NP).
  • Current licensure as a Nurse Practitioner or Physician Assistant in appropriate state.
  • Current certification in CPR
  • National certification, in applicable area.
  • Successful completion of Essentia Health credentialing process prior to practice.
