About The Position

The Nurse Practitioner (NP) in Hospice care will provide high-quality, person-centered end-of-life care to patients and their families. This role involves working independently and collaboratively with healthcare professionals, focusing on health assessments and decision-making skills. The NP will conduct home visits, support hospice team members, and ensure positive relationships with patients and caregivers.

Requirements

• Master's or Doctorate degree in Nursing from an accredited program.
• Current licensure as a Nurse Practitioner in MN and WI (or must apply for WI licensure within 3 months of hire).
• Current certification in CPR.
• National certification in applicable area.

Responsibilities

• Provide health care to individuals and families with a focus on end-of-life care.
• Conduct home visits to patients in various settings such as private homes and assisted living facilities.
• Rounding at the Solvay Hospice House.
• Support and educate hospice interdisciplinary team members virtually and in-person.
• Establish and enhance positive relationships with patients, caregivers, and co-workers.
• Participate in rotating evening and weekend call duties.
