About The Position

Habilitation/Companion/Transportation; Performs duties as delegated by Family Support Coordinator(s) in the community for persons with intellectual disabilities and/or behavioral health. Work with individuals on their individualized service plan (ISP) outcomes in their home or in the community.

Responsibilities

To participate in the ongoing communication regarding program planning and implementation for individuals.
Provide a safe, clean, comfortable and therapeutic environment during activities in the community.
Encourage, guide and train individuals in developing daily living skills and independence skills in accordance with their ISP.
Assist in organizing and participating in leisure/recreation activities.
