Responsible for training clients about the home visitation model and conducting outreach program activities for participants and their families in identified counties throughout Tennessee and surrounding areas to help improve health outcomes. Requirements

  • High School Diploma or Equivalent General Studies
  • Nurse Family Partnership Program 1-3 years
  • Proficiency in the Microsoft Office program or programs as indicated in the specific function within the department.
  • Ability to operate standard office equipment.
  • Ability to understand and compose complex written materials.
  • Ability to exercise independent judgment with tact and diplomacy in all situations.
  • Must be able to assess needs of participant and their families and respond appropriately.
  • Strong ability to integrate knowledge and theory into practice.
  • Ability to demonstrate skill in organization, coordination and time management.
  • Must be able to provide effective instructions to participants.
  • Must be able to communicate verbally in an effective and positive manner with participant and their families.
  • Must be able to demonstrate effective communication and negotiating skills with and between individuals and groups of people from all socioeconomic backgrounds, including Associates, social services providers, business entities, and students and their families.
  • Strong influencing, collaborative, coaching and consultative skills.

Responsibilities

  • Presents program information to patients, families, or community groups in order to promote good health outcomes and recruit clients into programs.
  • Provides program awareness to community partners.
  • Enters data, documents and evaluates the effectiveness of program activities.
  • Follows up with community referral sources and potential clients for recruitment.
  • Performs continuous quality assurance calls with new and existing program clients.
  • Performs other job functions as assigned or requested.
