About The Position

Carle Health is seeking an additional Advanced Practice Provider to join our Neurovascular team in Urbana, Illinois. Carle Health is a Comprehensive Stroke Center providing a full spectrum of neurovascular care. We have over 800 stroke admissions per year. The Neurovascular program at Carle consists of three physicians who are actively involved in neurointerventional procedures for aneurysms, stroke, AVMs, carotid disease, etc. We welcome candidates to join our busy and rewarding program to care for neurovascular patients. Requirements

  • Master's degree in Nursing or Physician Assistant.
  • Candidates must have at least 3 years of clinical APP experience.
  • Current Illinois registered nurse license and advanced practice nurse/physician assistant licensure

Nice To Haves

  • At least 1 year of clinical experience working with the stroke patient population (as APP, stroke navigator/coordinator, or RN) preferred.
  • Opportunity for faculty appointment available through the Carle Illinois College of Medicine for candidates with a strong academic interest

Responsibilities

  • Responsible for evaluating outpatients with TIA, ischemic and hemorrhagic strokes, in collaboration with the neuro vascular physician team.
  • Responsible for follow up on patients' status post endovascular cerebral procedures, both in the hospital and in the stroke clinic, in collaboration with the neuro vascular physician team.
  • Provides continuity of patient care by facilitating and coordinating communication between the health care team, the patient, and the patient's families
  • Develop and implement patient care plans, including discharge planning and coordination of care with other healthcare providers.
