About The Position

The coordinator of the Stroke Program will oversee the developmentand operation of the Primary Stroke Center. Prepares and submitsrequired administrative reports and serves as MyMichigan’s Primary Stroke Center liaison and works collaboratively with the American Heart Association, Joint Commission and other stroke accreditingagencies as required. This position applies nursing clinical practices and case management to patients who have suffered fromstroke by functioning as the Manager of the Stroke Program.

Requirements

  • BLS - Basic Life Support Required Equivalent Experience - Within 1 month of hire or transfer.
  • RN - Registered Nurse Required
  • Knowledge of Neuroscience nursing as normally acquired through 3-5 years of clinical experience working with neurology and/or neurosurgical patients.
  • Knowledge of general nursing principles, practice skills,organizational and regulatory functions required.
  • Demonstrated strong clinical leadership skills including excellentverbal and written communication.
  • Demonstration of effective team interaction and strong problem solving skill required.
  • Excellent customer service relations-commitment to community service education.
  • Proficiency with computer applications including Access, Outlook,Mysis, Excel and Work required.
  • Ability to work under pressure.

Nice To Haves

  • Marketing/speaking experience is preferred.
  • Masters in advancedclinical Practice preferred.
  • Credentials as a CertifiedNeuroscience Registered Nurse preferred.

Responsibilities

  • (30%)Collaborates and establishes rapport with the stroke patient’s physician(s), rehabilitation specialists, and community agencies.Evaluates inpatient and outpatient stroke patient documentation and intervenes with the appropriate medical providers to assurecompliance with established protocols and guidelines for caredelivery.
  • (30%)Assists Case Management in the monthly collection and compiling ofpatient stroke core measures data. Maintain database and analyzesdata to identify continuous quality improvement opportunities inaccordance with established performance measures and anticipated outcomes.
  • (30%)Reports stroke core measures and certification required data tocentral stroke registry. Assures department compliance with allaccreditation standards including Joint Commission and MichiganDepartment of Community Health.
  • (10%)Active member of the Stroke Review Committee. Oversees and conductsrequired meetings as directed by the Primary Stroke Center MedicalDirector. Reviews data, existing care protocols and programcompliance and collaborates with the stroke medical director.
  • Assists in the development and implementation of the stroke educational program to the medical staff working with strokepatients.
  • Provides appropriate and clear communication – both oral andwritten - to all customers.
  • Creates a climate of clinical excellence that results in improvedpatient care and optimal outcomes.
  • Maintains awareness of current national and accrediting bodytrends, practice changes and research findings related to the delivery of stroke care and shares this knowledge with clinical leaders.
  • Assumes other duties including being accountable for ongoing acquisition of neuroscience nursing knowledge and skills -especially in the area of stroke and applies accordingly.
  • Identifies opportunities and collaboratively works to implement newneuroscience programs and/or clinics based on identified needspresented by physicians, allied health professionals or patients.
  • Other duties as assigned.
