About The Position

The Neuro Rehab Licensed Master Social Worker at Hope Network is responsible for facilitating social work care plans during treatment episodes, ensuring clinical quality, and coordinating with various healthcare professionals. Requirements

• Master's degree in Social Work
• Three years of case management experience or related social work experience
• Full license (LMSW)

Responsibilities

• Facilitates the social work care plan for the treatment episode and monitors clinical quality.
• Coordinates efforts among various healthcare professionals including psychiatry, nursing, and therapy services.
• Facilitates communication between the treatment team and external parties such as Nurse Case Managers and insurance adjustors.
• Acts as an advocate and liaison for consumers' rights and benefits.
