About The Position

The Neuro Rehab Licensed Master Social Worker at Hope Network Neuro Rehabilitation plays a crucial role in facilitating social work care plans for individuals undergoing rehabilitation. This position involves coordinating with various healthcare professionals to ensure comprehensive care, advocating for consumer rights, and supporting timely discharge planning. Requirements

• Master's degree in Social Work.
• Three years of case management experience or related social work experience and full license (LMSW).

Responsibilities

• Facilitates the social work care plan for the treatment episode and monitors clinical quality.
• Coordinates efforts among psychiatry, physiatry, psychology, nursing, physical therapy, occupational therapy, speech language pathology, and therapeutic recreation.
• Facilitates communication between the treatment team and external parties such as Nurse Case Managers and insurance adjustors.
• Acts as an advocate and liaison for consumers' rights and benefits.
