About The Position

The Nephrology Inpatient Provider delivers consultative nephrology care to hospitalized patients, managing acute and chronic kidney conditions, dialysis needs, and fluid/electrolyte disorders. This role partners closely with hospitalists, intensivists, nursing, dialysis staff, and care management to ensure timely, evidence‑based, patient‑centered care.

Requirements

  • Acute Care Nurse Practitioner License or Physician Assistant license.
  • Experience in acute care or inpatient medicine.
  • Strong communication and consultative skills.
  • Ability to manage high‑acuity, fast‑paced inpatient environments.

Nice To Haves

  • Experience with CRRT, HD, PD, and general nephrology.
  • Familiarity with hospital EHR systems and dialysis documentation workflows.
  • Prior inpatient nephrology or ICU experience.

Responsibilities

  • Perform inpatient nephrology consultations and follow‑up visits.
  • Manage acute kidney injury, chronic kidney disease exacerbations, electrolyte abnormalities, acid–base disorders, and hypertension.
  • Oversee inpatient dialysis treatments (hemodialysis, CRRT, PD) in collaboration with dialysis nursing.
  • Provide recommendations to primary teams and document consult notes, progress notes, and care plans.
  • Respond promptly to consult requests from hospital medicine, ICU, surgery, and ED teams.
  • Communicate daily with multidisciplinary teams to align on patient status and treatment plans.
  • Coordinate with outpatient nephrology teams to ensure continuity of care at discharge.
  • Collaborate with case management and social work on discharge planning, dialysis placement, and follow‑up needs.
  • Maintain accurate, timely documentation in the EHR.
  • Follow hospital policies, CMS requirements, and dialysis regulatory standards.
  • Participate in quality improvement initiatives related to AKI, dialysis safety, and readmission reduction.
  • Support accurate coding and billing for inpatient nephrology services.
  • Participate in inpatient service coverage.
  • Provide education to residents, fellows, nursing staff, and other clinical teams, as appropriate.
  • Contribute to a culture of safety, collaboration, and patient‑centered care.
