If you're passionate about supporting patients with chronic respiratory conditions, empowering them through education, and helping reduce hospital readmissions, we’d love for you to join us as a Respiratory Disease Navigator. As a Respiratory Disease Navigator, you will be responsible for: Working directly with patients diagnosed with chronic respiratory diseases, including COPD/Emphysema, Asthma, Pulmonary Fibrosis, and other chronic pulmonary conditions. Supporting patients in becoming compliant with the management of their respiratory disease to improve quality of life and reduce readmission risk within 90 days. Collaborating with nurses, physicians, and therapists to ensure patients have a thorough understanding of: Their respiratory condition The purpose, usage, and importance of prescribed medications Available support services such as Pulmonary Rehab, Smoking Cessation, Support Groups, and Home Health resources Ensuring patients have appropriate resources, equipment, and support at home after discharge. Providing post‑discharge follow‑up and support through structured, routine phone calls. Following a systematic approach to visiting and supporting the respiratory patient population Monday through Friday. Performing RRT (Registered Respiratory Therapist) duties as needed.

Requirements

  • Associate's Degree in Respiratory Therapy (Required)
  • Master's Degree in Respiratory Therapy (Required)
  • 1+ year in Respiratory Therapy (Required)
  • 1+ year in computer system concepts and willing to learn new computer applications as needed (Required)
  • Respiratory Therapist (RRT) (Required)
  • Must obtain a Specialty Credential in COPD and/or Asthma (Required)
