About The Position

Plans of Safe Care (POSC) Navigator will conduct prenatal and postpartum hospital and home visits to women identified with substance misuse or a substance use disorder. Navigators are responsible for supporting families, conducting assessments, developing Plans of Safe Care and providing parent education in the areas of prenatal and postnatal care, breastfeeding, parent-infant attachment, child development, home safety and other maternal and child health related topics. They will also help identify participant strengths, needs, barriers and risks and link families to behavioral health, home visitation and other identified community resources for those needs. Navigator shall also provide and model empathetic support and feedback when working with new moms, babies and family members. This position is a part of a county-wide collaborative and will participate in data collection, meetings and trainings specific to the larger implementation of Plans of Safe Care in Los Angeles County.

Requirements

Basic advocacy skills.
Present ideas, information, and viewpoints clearly, both verbally and in writing.
Efficiently use the personal computer to include a variety or word processing software.
Utilize analysis, experience, and judgment to make decisions.

Provides services within a culturally sensitive, ethnic informed model of practice and is respectful to the community and participants.
Experience with substance abuse and an under-served population is required, SUD certificate preferred.
Bachelor's degree required.
Bilingual (English/Spanish) required.
A valid California driver's license is required for this position, as driving is an essential function of the role and is required for field work, client crisis response and emergencies. A current automobile insurance is required.
COVID-19 vaccination required. May request medical or religious exemption if applicable.

Responsibilities

Works closely with hospital social work team and hospital staff
Complete assessments and enroll participants bedside
Develop, in collaboration with family and hospital team, a Plan of Safe Care (POSC) for all families enrolled in services
Effectively conducts home visits with prenatal or postpartum participants
May be required to respond to participants during non-work hours
Develops trust and rapport with participants and provides support using empathic and family centered approach
Completes all needed documentation, including screenings, assessments and all POSC related documents within the timelines provided and in an organized manner, including clear and well written progress notes
Provides information to participants about maternal and infant health, parenting, attachment, home safety, infant behavior and other related topics using a participant and family centered approach.
Provide linkage and referrals to participants and their families in accordance with appropriate level of service; ensure all linkages and referrals are completed.
Coordinate support services, including but not limited to SUD treatment, home visitation and linking participants to auxiliary services and assisting participants and their families with accessing appropriate resources.
Coordinates support services for participants through participation in case conference and other related meetings with partner programs and agencies.
Coordinate and maintain appropriate service provision with referral source, i.e. Department of Children and Family Services or Community Agencies.
Collect required data and enter into an electronic database including updated community resources, participant information, participant contact, and generate reports.
Provide monthly reports to the program manager.
Respond to requests for information in a timely manner.
Maintain positive working relationships with local linkage and referral agencies.
Maintain a working knowledge of existing community programs and resources, the services provided and eligibility requirements.
Maintain caseload documentation in compliance with all agency policies and procedures as well as federal, state, county and local regulations.
Attends and participates in staff meetings, hospital meetings, grant meetings, supervision, and training as required.
