Plans of Safe Care (POSC) Navigator will conduct prenatal and postpartum hospital and home visits to women identified with substance misuse or a substance use disorder. Navigators are responsible for supporting families, conducting assessments, developing Plans of Safe Care and providing parent education in the areas of prenatal and postnatal care, breastfeeding, parent-infant attachment, child development, home safety and other maternal and child health related topics. They will also help identify participant strengths, needs, barriers and risks and link families to behavioral health, home visitation and other identified community resources for those needs. Navigator shall also provide and model empathetic support and feedback when working with new moms, babies and family members. This position is a part of a county-wide collaborative and will participate in data collection, meetings and trainings specific to the larger implementation of Plans of Safe Care in Los Angeles County.
