About The Position

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency operating throughout Arizona, providing residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. CBI is recognized as a premiere non-profit provider of substance use and behavioral health programs, utilizing cutting-edge, evidence-based models. The Navigator II's primary role is to enhance client success, identify and reduce barriers, and connect individuals with necessary services and resources. This role involves working with clients needing additional support and engagement, conducting outreach, engagement, case management, and providing ongoing wrap-around supportive services. The Navigator II is also responsible for mapping out appropriate next steps for individuals accessing various services to achieve ongoing success. This position is part of a multi-disciplinary team and requires documentation and tracking of patient interactions in accordance with Arizona Administrative Code (AAC), ADHS/DBHS Provider Manual, CARF standards, Community Bridges Policies and Procedures, and program-specific guidelines.

Requirements

  • Highschool diploma or GED is required
  • Minimum of 12 months of recovery from substance use and/or mental health disorders required
  • Minimum of ninety (90) days experience in a Navigator or Peer Support Specialist role required
  • Current AZ Driver's License (valid and in good standing)
  • Must be able to provide a 39-month MVR
  • Arizona Fingerprint Clearance card (program specific)

Nice To Haves

  • Associate degree (or higher) in a field related to Behavioral Health
  • 1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations
  • 1-3 years of substance abuse/mental health recovery-oriented service
