About The Position

The Navigator - Health and Well Being at Church World Service (CWS) in Fort Collins, Colorado, is responsible for assisting refugees and other eligible newcomers in accessing healthcare services and achieving self-sufficiency. This role involves conducting assessments, providing supportive services, and ensuring that clients are linked to necessary health resources. The position is hybrid, requiring a minimum of two days in the office, and focuses on facilitating the integration of refugee families into the community through health and well-being initiatives.

Requirements

• BA required
• Minimum of 3 years of relevant experience including working in case management with refugees.
• Highly detail-oriented with superior organizational and task management abilities.
• Respect for client confidentiality.
• Strong computer literacy with knowledge of word processing and database applications.
• Superior written and verbal communication skills.

Responsibilities

• Conduct trauma-informed intake and screening.
• Provide comprehensive family assessment using standardized instruments.
• Check eligibility for refugee cash assistance and other income supports.
• Provide supportive services to assist refugees in applying for and maintaining benefits.
• Connect with local service providers and community organizations to support client goals.
• Assess and monitor employment progress and modify resettlement plans as needed.
• Provide health-related services, including referrals to health screening providers and arranging transportation to appointments.
• Maintain accuracy in client case files and ensure compliance with reporting requirements.
• Perform other duties as requested to support team needs.
