About The Position

Requirements

  • Must have the ability to critical think and utilize appropriate resources.
  • Excellent communication skills in efforts to promote collaboration with the multidisciplinary team for the continuity of patient care.
  • High school diploma or equivalent
  • Minimum of four years experience working within a clinical health care environment
  • Oncology related education, counseling, planning, management or similar experience required

Nice To Haves

  • Medical Assistant (certified) experience with medical, radiation, and /or surgical oncology/hematology patient population preferred

Responsibilities

  • Provide assistance in two highly functioning support services within the Practice: Social Work and Patient Navigation.
  • Appointment notification and follow up calls for the Social Work team
  • Completes applications for patient resources
  • Work in Navigation Patient Tacker Smart Sheet to track patient identification to the Enhanced Oncology Model
  • Place follow up appointments on Navigators calendars and starting and ending performance periods for patients within the EOM program.
  • Provides medical literature, websites and support group information to patients and caregivers.
  • Assists patients in keeping follow-up appointments after treatment is completed.
