About The Position

As a Medical Case Manager at Fourth Street Clinic's MVP (Medically Vulnerable People) program in Sandy, you play a pivotal role within the MVP team, serving as a compassionate advocate for patients in need. Working onsite at our MVP program in Sandy, for the unhoused and medically vulnerable individuals in our community, you will collaborate closely with the healthcare team to identify vulnerable patients and conduct thorough needs assessments. With a patient-centered approach, you will create and maintain care plans tailored to individual needs, coordinating medical, behavioral health, and support services both within the clinic and with community partners. Your dedication to continuity of care, accessibility to services, and addressing social determinants of health will ensure that patients receive comprehensive support as they navigate their healthcare journey while staying at MVP.

Requirements

  • Minimum education - High School Diploma or its equivalent.
  • Ability to be detail-oriented and work to a high level of accuracy in every aspect of the job.
  • Ability to work well with a team and demonstrate a caring and compassionate commitment to each patient and their unique circumstances.
  • Demonstrate high interpersonal skills, time management skills, and an ability to meet established productivity standards.
  • Ability to work collaboratively with multiple health professionals in a busy and complex environment using tact, diplomacy, and discipline.

Nice To Haves

  • Bachelor's degree in a healthcare-related field.
  • 2 years of experience in case management, or other related medical patient support functions in an ambulatory care setting.
  • 400 hours of experience working, volunteering, or interning in the Case Manager realm.
  • Proficiency in Microsoft Office Suite and EPIC electronic health record system (EHR)
  • Preferred proficiency in Microsoft Office Suite and EPIC electronic health record system (EHR)

Responsibilities

  • Provide medical case management to a designated caseload of 25-34, aligned with FSC Medical Case Management services and expectations.
  • Conduct a needs assessment for patients identified as medically vulnerable.
  • Develop and update patient-centered care plans that build upon the areas of need identified by the case management needs assessment.
  • Coordinate the delivery of prescribed medical needs.
  • Facilitate transport for supportive services for patients within the clinic and with community partners.
  • Collaborate with other service providers to ensure patient continuity of care.
  • Maintain knowledge of clinic policies, procedures, systems, and electronic health records.
  • Meet medical and data documentation expectations.
  • Assist in creating a healthy work environment that endorses FSC's values and standards.
  • Support and encourage a culture of collaboration that values mutual respect, partnership, learning, trust, and fairness.
