About The Position
The Nurse Navigator position will provide guidance and support to dementia patients and their caregivers within the newly formed South Carolina Alzheimer's Network (SCAN). SCAN is a multi-site clinical program at MUSC Health designed to provide patients with cognitive disorders an early and accurate diagnosis and treatment plan, longitudinal co-management with primary care, access to the new Alzheimer's disease medications, and access to the latest research. This full-time, on-site position reports directly to the Lead Dementia Care Navigator. This position actively supports SCAN patients and their families in navigating the complexities of the dementia related diagnosis, including the treatment process, available resources, education, crisis prevention, and care coordination.
