As a Clinical Pharmacist at Cleveland Clinic, you will play a critical role in delivering safe, effective, and patient-centered medication management within a high-acuity hospital setting. In each of these three available positions, you will have the opportunity to apply your expertise in one of several highly specialized areas: Transplant, Emergency Medicine, or Critical Care. Whether providing care for complex transplant patients, delivering critical guidance in the fast-paced Emergency Department, or managing therapies for critically ill patients, you will play a vital part in advancing patient outcomes. This posting represents multiple openings, and selected candidates will be hired into a specific specialty area based on interest, experience, and departmental need.

Requirements

  • PharmD or Bachelor's of Science in Pharmacy from an accredited College of Pharmacy.
  • Current state licensure as a Registered Pharmacist (RPh) or eligible in current state within 90 days of hire.
  • Current Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross.
  • Completion of a Board of Pharmacy approved course in the administration of immunizations or certification within 90 days of hire for Individuals administering immunizations.
  • Current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) Certifications through the American Heart Association (AHA) upon or within one of hire for individuals responding to adult or pediatric cardiopulmonary arrest events.

Nice To Haves

  • Board Certification.
  • Completion of a Post-Graduate Year 1 Residency (PGY1), fellowship or relevant post-graduate pharmacy experience.
  • Post-Graduate Year 2 Residency (PGY2) or relevant experience for Clinical Pharmacists in a specialized pharmacy practice area.

Responsibilities

  • Provide safe, efficient, patient-centered and evidence-based pharmacy and medication services.
  • Ensure the appropriate selection, dosing, administration and dispensing of medication orders.
  • Verify medication orders and/or dispense medications accurately according to state and federal regulations.
  • Document services in a complete, accurate and timely manner.
  • Actively contribute to medication safety initiatives.
  • Ensure all pharmacy operations are in compliance with national accreditation standards.
  • Educate and train students, residents, other caregivers, and/or patients.
  • Provide cost-effective care.
