Hennepin Healthcare has a 1.0 FTE (80 hour per two week pay period) opening for an Ambulatory Care Pharmacist in our HHS Ambulatory Care/MTM Pharmacy Department. This position is in person and will work day shifts, no weekends. This position will support our MTM program by serving in an existing clinic or may serve in an expansion clinic, depending on departmental needs. HHS MTM services are delivered in both primary care and specialty care, and this role will likely involve a combination of these 2 practice settings. This pharmacist will be responsible for providing Comprehensive Medication Management (CMM) in all clinic settings. This pharmacist also will be credentialed and privileged through the Credentialing department at HHS. SUMMARY Works collaboratively with the patient and care team to ensure medications are indicated, effective, safe, and patients are adherent with therapy. Maintains standards set by the department and translates policies and procedures into the daily operation of the pharmacy and ambulatory clinic. Responsible for acting as an information resource to patients, physicians, nurses, and other healthcare professionals regarding medication therapy and drug-related information and literature. Plans and organizes work schedule of supportive personnel (ex. interns, students) and pharmacists, as required.
