About The Position

The MSW Social Worker - Hospital Care Management position is a part-time role focused on providing social work treatment for patients in the Emergency Department and hospitalized patients at Denver Health. The role involves assessing the biopsychosocial needs of patients and their families, creating individualized care plans, and providing clinical interventions to support their health and well-being. The position requires collaboration with multidisciplinary teams to ensure comprehensive care and effective discharge planning.

Requirements

  • Master's Degree from a CSWE accredited college or university is required.
  • 1-3 years of experience, typically one year of full-time post-master's degree Social Work practice under the supervision of a Licensed Clinical Social Worker.
  • Knowledge of coordination of care/discharge planning, crisis intervention, and supportive counseling.
  • Knowledge of medical/behavioral health terms, abbreviations, and diagnoses, demonstrated verbally and in writing.
  • Skill in problem-solving and use of critical thinking skills.
  • Skill in providing care appropriate to the age of the patients served within the healthcare setting.
  • Skill in establishing a treatment alliance and engaging the patient in goal setting and prioritizing.
  • Ability to manage the professional relationship including termination and/or transition of care.
  • Ability to respond to crisis situations and provide support.
  • Ability to advocate in a professional manner as appropriate.

Responsibilities

  • Conduct comprehensive biopsychosocial assessments that are thorough, individualized, objective and complete.
  • Provide intervention appropriate to the presenting problem, utilizing key case management components including assessment/reassessment, care planning, coordination of care, and documentation.
  • Evaluate emotional components and assist patients in identifying strengths and dysfunctional factors to establish detailed discharge/transition of care plans.
  • Provide care coordination throughout the continuum of care and/or hospitalization from case referral to case closure.
  • Apply knowledge of individual, group and family functioning to ensure appropriateness of treatment methods to assist patients in achieving care plan goals.
  • Demonstrate the ability to develop effective brief interventions that are socio-culturally sensitive.
  • Consult, negotiate, and coordinate with internal and external resources to assure collaborative efforts to maximize patient outcomes.
  • Facilitate multi-disciplinary communications and patient care meetings, ensuring patient involvement in care planning and goal setting with informed consent documented.
  • Demonstrate knowledge of major concepts, principles, and theories of normal and abnormal human development, and its application to specific age groups.
  • Assist clients to maximize adjustment to illness/disability/trauma by addressing biopsychosocial issues and enhancing problem-solving and coping skills.
