About The Position

Under the supervision of the Emergency Services (ES) Program Manager, provides crisis intervention, outreach services in the home, office and community settings, and case management to adults experiencing behavioral health crises. Collaborates with psychiatric and allied mental health team in assessment, diagnosis, intervention and treatment planning. Requirements

  • Associate’s degree in a human service field or a certified peer specialist
  • At least 2 years experience working with in a mental health or school setting
  • Knowledge of human development and family systems issues.
  • Understanding of mental health and substance use diagnoses
  • Knowledge of community resources for clients and families with behavioral health disorders.
  • Advocacy and coaching skills.
  • Valid driver's license and use of in order to perform the major functions of the job.
  • Automobile liability insurance coverage for vehicle being used in accordance with Center requirements.
  • Ability to work collaboratively as a team to coordinate care for client with intensive behavioral health needs.

Responsibilities

  • Assists in conducting risk screenings for adult clients in crisis in clinics, in homes, and in the community.
  • Provides individual interventions to keep the client stabilized as identified in the ES Mobile Team, in the home, at a Center facility, and in the community.
  • Provides case management interventions such as resource linkage, skills building, support, and interventions to clients following a behavioral health crisis to support stabilization.
  • Coaches clients in symptom management strategies.
  • Supports clients in educating them around medication management.
  • Maintains knowledge of available providers and resources in the community to address specialized needs.
  • Participates in supervision, team conferencing, department and agency meetings.
  • Maintains detailed, accurate, and timely documentation in medical record.
  • Collects, records, and analyzes required data within scope of job responsibilities.
  • Enhances and maintains professional expertise through educational activities and trainings.
  • Engages in trainings on evidence-based practice models specific to their assigned population, including motivational interviewing and cognitive behavioral therapy, implementing learned skills in daily interventions.
  • Meets requirements for client contact time according to treatment plan, and for agency productivity targets.
  • Follows agency policies and standards for client rights and confidentiality.
  • Other duties as assigned
