About The Position

The Mobile Services Family Nurse Practitioner / Physician Assistant plays a vital role in advancing Golden Valley Health Centers ’ mission by bringing compassionate, high-quality primary care directly into the communities we serve. Through autonomy in practice, strong teamwork, and a deep commitment to health equity, the Mobile Services NP or PA helps remove barriers to care, improve health outcomes, and build trusted relationships with underserved populations across our region. Location and Schedule: Mobile Medicine Unit will support Stanislaus County, and support Ceres Whitmore when hours are completed. Schedule is Monday – Friday, 10:00am – 7:00pm, with flexibility to work before or after those hours. Requirements

  • Ability to travel to sites/locations which can vary upon weather conditions and needs of the community, and special functions as needed
  • Valid California Driver’s License, acceptable driving record and vehicle insurance.
  • Strong interpersonal skills and ability to work as a team member.
  • Demonstrates expertise in addressing the physical, mental, and social needs of vulnerable members of the community.
  • Knowledge of electronic health records, Epic preferred
  • Graduation from an accredited school of registered nursing and Nurse Practitioner program, or an accredited school for Physician Assistants.
  • Minimum two (2) years working experience as a Nurse Practitioner, or Physician Assistant.
  • Furnishing License, Current DEA Certification, and board certification.
  • Current BLS CPR recognized by the American Heart Association or American Red Cross.
  • Must be able to obtain outpatient privileges.

Responsibilities

  • Consistently adheres and/or exceeds GVHC’s communication guidelines and expectations with patients, peers, and supervisors.
  • Exemplifies professional and welcoming communication through respectful language, positive tone, appropriate body language, and consistent eye contact.
  • Demonstrates strong active listening skills, showing empathy and understanding while responding to patient and team needs with professionalism and a commitment to exceeding expectations.
  • Obtain comprehensive medical histories, perform physical examinations, for patients across all age ranges, from pediatrics to geriatrics, with a wide variety of presenting concerns/conditions.
  • Orders and interprets laboratory, imaging, and other diagnostic tests appropriate to patient care needs in community and mobile care settings.
  • Perform or assist with point-of-care testing, specimen collection, and other screening procedures conducted on-site in the mobile unit as needed.
  • Assesses, diagnoses, and manages common primary care, developmental, behavioral, and psychosocial health concerns in individual patients and/or family members, following established protocols, supervision agreements, and best practices.
  • Maintain accurate and up-to-date patient records, including problem list, medication list and immunization histories, using the electronic health record system.
  • Promote health and disease prevention through age-appropriate screenings, immunizations, counseling, physical examinations, and accepted screening procedures.
  • Observe all existing Golden Valley Health Centers medical protocols, personnel policies and Principles of Practice
  • Maintain a current Basic Life Support (BLS) CPR, certified by the American Heart Association or American Red Cross.
  • Actively participate regularly in quality assurance and peer review activities, as well as medical and general staff meetings.
  • Prescribes and administers medications safely and appropriately in alignment with standardized procedures, collaborative agreements, and mobile unit protocols, adapting to the unique needs of patients in the community.
  • Delivers high quality, patient-centered care that prioritizes safety, compassion, and clinical excellence, while maintaining productivity standards that supports organizational goals and community health outcomes.
