About The Position

Chartiers Center is a private, non-profit corporation funded by state, county, 3rd party insurances and private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day and recreational services for adults with intellectual disabilities, homeless outreach and housing program. FULL TIME MONDAY- FRIDAY Daylight developing relationships with Providers in the Community. NO weekends, Holidays or On-Call shifts As a Mobile Registered Nurse with our ASC Program at Chartiers Center, you would be committed to providing compassionate integrated health support to people facing mental health challenges with a community-based integrated care approach. This position will maintain a caseload of individuals who experience: - An acute incident of a health condition - A chronic health condition that is unstable or progressive - A lack appropriate physical/behavioral/substance health care - Will benefit from health coaching and/or lack appropriate supports with and linkage to health care providers.

Requirements

• Registered Nurse (RN) Certification from an Accredited Institution with PA licensure with 3 years behavioral health experience
• Valid driver's license

Responsibilities

• Developing engaging and positive relationships with individuals and supporting them with their self-identified goals.
• Utilizing evidence-based practices related to Wellness coaching, and engaging individuals in managing their own care to achieve their desired goals
• Providing services and supports in the location consistent with the needs and situation. May include travel to other health providers, in the home/community of the individual served and may also include facilitating transportation using an agency vehicle to medical appointments.
• Analyzing individual and aggregate BH and PH care utilizations data from BH and PH Managed Care Organizations on these individuals as abasis for developing wellness plans.
• Developing and distributing wellness materials for consumers identifying and managing chronic health conditions.
• Managing the collection and reporting of outcomes data and preparing related reports and other documentation as required.
