We are seeking an experienced Physician Assistant (PA-C) to join our growing Mobile Primary Care team serving seniors in home-based care, assisted living (ALF), skilled nursing facilities (SNF), and senior living communities . This is not a traditional clinic role. This is community-based, field-based medicine focused on geriatrics, chronic disease management, dementia care, and value-based care delivery. If you are a Physician Assistant passionate about senior care, panel management, and independent practice , this role offers autonomy with strong multidisciplinary support. Requirements
- Active, unrestricted Physician Assistant (PA-C) license
- 1–2+ years of clinical experience in primary care or geriatrics
- Strong clinical judgment and comfort practicing independently
- Interest in leadership, panel growth, and clinical program development
- Valid driver’s license (field-based/mobile role)
Nice To Haves
- Experience in home-based care, SNF, ALF, or senior living communities preferred
Responsibilities
- Deliver Mobile Primary Care in home and senior living settings
- Provide comprehensive geriatric assessments and chronic disease management
- Manage hypertension, diabetes, CAD, CVA, dementia, and complex Medicare populations
- Coordinate care with interdisciplinary teams across hospice, palliative, and behavioral health
- Drive patient engagement, retention, and outcomes within a value-based care framework
- Practice independently while maintaining high-quality documentation and follow-through
