Requirements

  • Active Minnesota Physical Therapist license
  • Valid driver’s license and dependable transportation
  • Home health experience a plus, but not required—outpatient and inpatient PTs encouraged to apply
  • Passion for empowering patients through one-on-one, goal-driven care

Responsibilities

  • Perform in-home physical therapy evaluations and mobility assessments
  • Create individualized treatment plans centered on safety, strength, and functional progress
  • Deliver hands-on therapy, gait training, and home exercise instruction
  • Recommend equipment and home modifications to enhance independence
  • Collaborate with nurses, caregivers, and interdisciplinary providers
