About The Position

Mobile Mental Health Coordinator (MMHC) (Full-time, 40 hours) Exempt BUILD, Inc. (Broader Urban Involvement and Leadership Development) is presently seeking a committed, self-motivated, and innovative individual to join our team. Mobile Mental Health Coordinator (MMHC) is responsible for the successful implementation, on-going program development and day-to-day maintenance of services for BUILD’s MMH program; duties include but are not limited to maintaining compliance in the Apricot/CIS databases, coordinating services within the team of clinicians and case managers, and assisting in the management of the program budget. In addition, the MMHC will provide assessment, referral, and follow-up service to youth and families when needed. The candidate must be self-directed, have team management skills, strong communication skills, strong project management skills, and a consistent focus on health and wellness. Candidates must be able to work with a diverse workforce in worksite settings, while keeping up on mental health trends, organization communication plans, and supporting overall program delivery needs. The MMHC reports directly to the Manager of Mobile Mental Health (MMMH).

Requirements

  • A minimum of 3 years of direct experience working with inner-city youth in community-based services, or with community-based programming or a bachelor’s degree in social work, Education, Youth Development, Nonprofit Management, or related field
  • Required: Must be able to drive the Mobile Mental Health Bus.
  • Required: 1-2 years experience collaborating and coordinating events for youth and community
  • Must be a team player and is open to work with others and communicate effectively.
  • Must possess a valid driver's license and be able to operate a large vehicle.

Responsibilities

  • Effectively addressing both physical and mental health needs of youth and families through: a) ongoing training of staff to identify client needs b) in-field support and consultation with other professionals; c) initial needs assessment and referrals; d) establishing referral partners and identifying opportunities for more formal collaboration.
  • Assist MMH Manager in managing aspects of the MMH Program; including grant deliverables and budget oversight. Strategic and long-range planning and performance
  • Support in planning, coordination, and delivery of programs and initiatives.
  • Collaborates with and provides leadership to internal and external experts in developing new programs and services that are responsive to changing populations and community needs; and to achieve appropriate service integration.
  • Performs other job-related duties as requested, required and/or assigned.
  • Submit all required documentation, as requested including, but not limited to assessments, evaluations, progress reports, etc.
  • Participate in agency wide events.
  • Coordinate community mental health events and pop-ups.
  • Manage external partnerships and resources. Internal staff management and development

  • Work with team and front-line staff to ensure effective and efficient program delivery.
  • Maintains knowledge current in the field.
  • Maintains knowledge current on agency’s theories, methodologies and best practices.
  • Attends departmental and agency-wide staff meetings, as scheduled and agency-wide events as required.
  • Supervises maintenance of all reporting databases including Apricot and CIS and filing system that tracks program activities and participant records.
  • Collaborate with the Medicaid Billing Specialist to ensure all data entry is complete and accurate.
  • Coordinate the delivery of services among different program activities to increase effectiveness and efficiency. Direct Service
  • Maintain a small caseload based on supervisor recommendations.
  • Receives case assignments, establishes treatment plans, reviews case progress and determines case closure.
  • Makes referrals in accordance with service plans to other organization units and external providers.
  • Develop and facilitate psychoeducation and/or life skills workshops.
  • Participates and contributes in regular supervision meetings, team meetings, in house training sessions and conferences.
  • Provides immediate advocacy, crisis intervention and problem solving in assigned areas.
  • Develop and manage partnerships in identified neighborhoods.
  • Coordinate internal wrap-around services.
  • Maintains documentation of client’s individual and group services
  • Attend departmental and agency-wide staff meetings, as scheduled and agency-wide events as required.
  • Perform all other duties as assigned by leadership to support the evolving needs of the MHTS department and the agency.
  • Support Individuals that are at increased risk of experiencing trauma-related instability, recidivism, or community violence in stabilizing, thriving, and setting and accomplishing goals
  • Support MHTS Programs in coordinating outings and meeting with partners that will better suit clients
