About The Position
Stepping Stone Behavioral Health is seeking dedicated and compassionate Licensed Social Workers (LSW/LISW), Licensed Professional Counselors (LPC/LPCC), or Licensed Chemical Dependency Counselors (LCDC II, LCDC III, LICDC) to join our outpatient behavioral health team . This mobile clinician role provides mental health and substance use counseling directly inside skilled nursing and long-term care facilities , offering meaningful support to residents coping with emotional, behavioral, and substance-related challenges.
Requirements
- Must be 21 years or older with a valid driver’s license
- Active Ohio license : LSW, LISW, LPC, LPCC, LCDC II, LCDC III, or LICDC.
- Ohio Medicaid Billing Number and NPI Number required (or ability to obtain).
- Minimum of an Associate’s Degree (Bachelor’s or Master’s preferred).
- Strong interpersonal, organizational, and documentation skills.
Responsibilities
- Conduct bio-psychosocial assessments and develop individualized treatment plans.
- Provide individual and group counseling , teaching coping and recovery skills.
- Identify resident needs, set measurable goals, and guide them toward emotional stability.
- Participate in treatment team meetings and collaborate with facility staff and community partners.
- Maintain timely and accurate clinical documentation in EMR systems.
- Assist with telehealth sessions and submit required daily reports.
- Facilitate safe discharge planning and coordinate aftercare services.
- Uphold confidentiality, professionalism, and compliance with facility and state regulations.
