About The Position

Glade Run's Psychiatric Mobile Medication Program serves Butler County adults 18 years or older with a psychiatric diagnosis, a history of inpatient hospitalizations, and difficulties in their progress towards recovery. The program provides the education, support, and skill development needed for individuals to become independent and responsible for their medications. Who We Are: Founded in 1854 by the Rev. Dr. William Passavant in Zelienople, Pennsylvania, Glade Run has evolved over the last 170 years to meet the changing and increasingly challenging needs of children, adults and families throughout Western Pennsylvania and beyond. Last year, Glade Run served over 7,000 individuals with impactful, innovative services in school, home, and community based settings. Requirements

  • Bachelor’s degree in Social Services or other related field.
  • Minimum of one year of supervisory experience in the mental health or physical health profession preferred.
  • At least one year of experience working with seriously mentally ill patients.
  • Must have a valid driver’s license, acceptable driving record
  • Ability to drive for work purposes including to client homes and to job sites throughout Butler County.
  • Must be able to adjust hours to job requirements.
  • Ability to implement agency policies and programming including but not limited to the following: Medication adherence, Medication administration, Medication reconciliation.
  • Ability to make sound judgements regarding client care.
  • Must have acceptable PA State Police clearance, PA Child Abuse History Clearance, FBI Clearance, and Arrest/Conviction Report and Certification Form.
  • Must be able to identify with the agency Mission Statement and support the agency’s Core Values.

Responsibilities

  • Responsible for providing oversight and monitoring of client medications; education regarding medications; linkage with appropriate resources and supportive case management services.
  • Assists in the development and administration of the policies and procedures necessary for the operation of the Mobile Medication Program.
  • Participates in the agency hiring process and supervision of the Mobile Medication Program staff.
  • Supervises mobile medication staff in the completion of direct service activities.
  • Lead, motivate, and coach others in a fast-paced environment.
  • Provide coaching and feedback to employees on performance related issues
  • Ensures financial integrity of the program via coordination of managed care authorizations, schedule of service delivery and staff coverage.
  • Collaboration with stakeholders and other providers for linkage to appropriate resources and supports.
  • Participate in Quality Improvement activities to enhance positive consumer outcomes.
  • Schedules adequate staff coverage for the Mobile Medication Program.
  • Provides regularly scheduled group and/or individual supervision of mobile medication staff.
  • Ensure medications are stored and administered in accordance with all applicable regulatory entities.
  • Order and ensure that all medications on site have proper medication orders present and are accurate.
  • Performs medication counts: Daily for controlled substances, weekly for all other medications. Reports discrepancies immediately to supervisor, completes all corresponding documentation
  • Provides oversight in the referral process; development of treatment plans, treatment reviews and other related documentation of the mobile medication program.
  • Attends staff meetings, conferences and in-service training as required.
  • Attends treatment planning sessions and treatment reviews as a member of the interdisciplinary treatment team.
  • Coordinates with medical entities and pharmacies related to Mobile Medication Client participation.
  • Maintains a small caseload of clients.
  • Assists in program referrals to include admissions and service coordination.
  • Collaborates with various program supervisors to ensure expeditious service delivery to clients.
  • May be required to perform services in clients’ homes
  • Performs other related duties as may be required or requested.
