Requirements

Valid NYS Paramedics Certification
3 years of experience in 911, Interfacility or Community Paramedicine
BCLS, ACLS, NYS Paramedic. Valid NYS drivers license
Daily use of PPE and electronic documentation.
Frequent, prolonged standing, walking, and patient/equipment transfers.
Ability to carry a 30–40 lb backpack during patient visits.

Responsibilities

Conduct in -person home visits to manage acute illnesses and injuries.
Provide treatment in line with evidence -based guidelines and collaborate with a multi -disciplinary team, including telehealth Emergency Physicians and care coordinators, to handle complex cases.
Identify risk factors, such as medication compliance, limited mobility, impaired nutrition, fall safety, and social isolation.
Maintain patient confidentiality, complete timely electronic records, and adhere to acute care standards.
Respond on an on -call basis using a personal vehicle, with the option to use a Care2U vehicle when available and pick up necessary equipment from designated locations.
