About The Position

Carle Health is seeking an experienced Advanced Practice Provider (APP) to join our Mobile Health Clinic (MHC) in the Bloomington-Normal, Illinois area. In this challenging and rewarding role, the APP works with a team of physicians, APPs, nurses, medical assistants, social workers, and other supportive team members to address common, urgent, and complex care issues. Full-time and part-time positions are available, and new graduate Nurse Practitioners (NPs) and Physician Assistants (PAs) are encouraged to apply. The APP will provide comprehensive, patient/family-centric care to people of all ages within the community, diagnosing and treating acute health care problems such as illnesses, infections, and injuries. Responsibilities also include wellness care, physicals, immunizations, chronic condition screening and education, point of care lab testing and interpretation, and community outreach activities. The APP will work collaboratively with community partners and must be knowledgeable in child and adult immunization schedules and basic Women's Health. The schedule is flexible to support community events and will typically be Monday to Friday from 9:00 AM to 5:00 PM, with one weekend per month and occasional evening shifts in underserved neighborhoods. In the event the MHC is not in operation, work may be reassigned to support a Carle convenient care location. The role requires a deep understanding and respect for the community and the people served.

Requirements

  • Master's degree in Nursing or Physician Assistant
  • Two years of clinical practice preferred
  • For new graduates, a robust nursing background and extensive training preferred
  • Willingness to obtain CDL
  • Experience providing care/support to all age groups and underserved populations
  • Public speaking skills
  • Current Illinois registered nurse license and advanced practice nurse/physician assistant licensure

Responsibilities

  • Provide comprehensive, patient/family-centric care to people of all ages within the community
  • Diagnose and treat acute health care problems such as illness, infections, and injuries
  • Conduct wellness care, physicals, and immunizations
  • Maintain the integrity of the VFC program
  • Screen and educate patients on chronic conditions
  • Perform point of care lab testing and interpretation
  • Screen for social and environmental determinants of health
  • Facilitate linkages to primary care and other community resources
  • Engage in community outreach activities
  • Establish collaborative relationships with community partners
  • Demonstrate knowledge in child and adult immunization schedules and basic Women's Health
