About The Position

Carle Health is seeking an experienced APP to join our Mobile Health Clinic (MHC) in the Bloomington-Normal, Illinois area. Full-time and part-time positions are available, and new graduate NPs/PAs are encouraged to apply. The role involves working with the MHC team to provide comprehensive, patient/family-centric care to people of all ages within the community. Responsibilities include diagnosing and treating acute health care problems, providing wellness care, chronic condition screening, point of care lab testing, and linking patients to primary care and community resources. The position requires collaboration with community partners and knowledge of immunization schedules and basic Women's Health. A flexible schedule is necessary to support community events, with hours typically from M-F / 9:00a-5:00pm, one weekend per month, and occasional evening shifts. In the event the MHC is not operational, work may be reassigned to support a Carle convenient care location. The role demands a deep understanding and respect for the community served.

Requirements

  • Master's degree in Nursing or Physician Assistant
  • Two years of clinical practice is preferred
  • For new graduates, a robust nursing background and extensive training preferred
  • Must be willing to obtain CDL
  • Current Illinois registered nurse license and advanced practice nurse/physician assistant licensure

Responsibilities

  • Provide comprehensive, patient/family-centric care to people of all ages within the community
  • Diagnose and treat acute health care problems such as illness, infections, and injuries
  • Provide wellness care, physicals, and immunizations while maintaining the integrity of the VFC program
  • Conduct chronic condition screening and education
  • Perform point of care lab testing and interpretation
  • Screen for social and environmental determinants of health
  • Facilitate linkages to primary care and other community resources
  • Engage in community outreach activities
