CAN Community Health is now hiring a Mobile HCV Community Health Worker I Schedule: Full-Time | Day Shift | Monday-Thursday 8:00 am - 5:00 pm Friday 8:00 am - 12:00 pm Salary: $21-$24 per hour based on experience Are you passionate about patient care and ready to make a difference every day? Tuition Reimbursement Plan Other voluntary plans are available to support you and your family Career growth opportunities in a supportive environment What You’ll Do The Mobile Community Health Worker provides patient-centered HIV/STI/Hepatitis C testing and health education; supports clinical care on mobile and clinic-based units; delivers risk reduction counseling and PrEP/Hepatitis C linkage; and conducts community outreach to expand access to prevention and treatment. This role focuses on direct service delivery in outreach environments while ensuring that all documentation and practices comply with professional and legal standards. Promotes and practices CAN Community Health Inc’s mission, vision, guiding principles, and values and follows its policies and procedures. Participates and strives to achieve assigned Annual Performance Objectives as established by CAN Leadership Maintain confidentiality and comply with HIPAA and CAN Policies. Adhere to legal, ethical, OSHA/Safety, and infection control standards. Complete required trainings and maintain certifications (e.g HIV testing, CPR). Conduct medical intake and prepare patients for examinations on mobile or clinic-based units. Prepare chart to include all diagnostic documentation as needed for patient care. Assist providers during exams and explain procedures to patients. Performs Rapid Tests for Pregnancy, UTIs, STIs. Maintain infection control/safety procedures, temperature logs and clean/sterilize rooms. Requirements

  • High school diploma required
  • Basic Cardiac Life Support (BLS) Certification required
  • Must be able to operate a motor vehicle and have valid insurance and driver’s license.
  • Must be able to pass a Level I and Level II Background check as required
