About The Position

Under indirect supervision, provides, oversees, and/or administers a wide range of psychosocial evaluation/assessment, diagnostic, crisis intervention services in a high-intensity clinical environment which requires a high degree of independent decision-making. Most aspects of this job require clinical assessment and evaluation skills to consumers that report in crisis to a variety of community settings including magistrate’s office, community sites, and DAYMARK Service Areas in crisis. Case management/supportive services for disposition of cases is also a core function of this job. Provides psychoeducation as indicated in the PCP Assists with crisis interventions Assists the team leader with behavioral and substance use disorder interventions Assists with the development of relapse prevention and disease management strategies Participates in the initial development, implementation, and ongoing revision of the PCP Communicates the beneficiary’s progress and the effectiveness of the strategies and interventions to the team leader as outlined in the PCP

Requirements

  • Must fall under one of the two tech levels below AND be able to demonstrate 1 year of experience providing crisis care in assertive outreach, assertive community treatment, emergency departments, inpatient units, or other services providing comprehensive emergency/crisis services.
  • Para Professional services are delivered by staff who have the life experience of being diagnosed with a serious mental illness or substance addiction problem, and are currently in recovery, stable, and living without relapses. The Paraprofessional has a high school diploma, or GED and at least five years of life experience with a serious mental illness or substance addiction.
  • Peer Support - Requires Peer Support Certification.
  • AP -requires a bachelor’s degree in social work, Counseling or another Human Services field with no experience.
  • QP- requires an associate or bachelor's degree in social work, Counseling or another Human Services field. Qualified Professional (QP) required.

Responsibilities

  • Ensures that consumer/legally responsible person receives information regarding rights and assistance as needed in exercising rights.
  • Assists consumer/legally responsible person in obtaining information about available service providers.
  • Helps the consumer make informed independent choices and develop a network of supportive individuals.
  • Assist the consumer with using the crisis plan and provide relapse prevention support.
  • Supports the consumer to maintain stable housing, improve housing situation, and enhance independent living skills.
  • Assists the client in gaining information about going back to school or job training
  • Developing and implementing a Mental Health Wellness Recovery Action Plan (WRAP)
  • Skill maintenance and enhancement around meals and social activities to build peer relationships where eating is not the core activity.
