We are offering a Monday-Friday schedule for our Critical Intervention – Care Manager position being offered in Essex County, NY. Requirements

• Bachelors accepted, Masters preferred with at least one year experience in the field of Mental Health and/or Social Services.
• One year Care Management experience in the field of Mental Health with a focus on youth, families and community engagement preferred.
• Experience working with youth, community, and families preferred.

Responsibilities

• Provide time limited, phase-based intensive case management services to assist complex youth between 11-17 years of age.
• Facilitate networking with community mental health partners to ensure proper referrals and follow-ups occur.
• Educate and work with families, caregivers, natural supports and community partners to stabilize youth in their community and at home.
