The position involves overall performance and implementation of Teen Reach programming. The individual will assist the Teen REACH Program Coordinator by formulating and coordinating individual needs assessments, goal setting, and conducting face-to-face and phone conversations on a weekly or bi-weekly basis. The role includes family engagement, linkage with community agencies, and follow-up services for youth and participants as needed. The individual will work to assist youth in determining the factors that put them at risk of engaging in or becoming victims of aggressive behavior and other anxieties. They will help identify methods for youth to avoid risk-taking factors and reach their goals. The position also requires reaching out to and re-engaging youth with inconsistent attendance, working with families to bring resources to the home, and connecting with programs. Additionally, the individual will conduct data collection and coordinate reporting to funders, marketing staff, and other stakeholders, utilizing the Illinois Department of Human Services (IDHS) required eCornerstone internal data system to ensure program data are properly entered and exported for program and administrative needs. The role provides support and program leadership for youth ages 11-17 who participate in Teen REACH services, along with other duties as assigned.
