About The Position

The Mental Health Specialist/Case Manager IV reports to the Program Director and is responsible for providing specialized services to clients in gaining access to needed medical, social, housing, economic, educational, and other direct services.

Requirements

  • Bachelor’s degree in social work or counseling in a related field.
  • One (1) year experience working in a similar or related field (preferred).
  • Communicate effectively in written and spoken English.
  • Work effectively with diverse ethnic, religious, socioeconomic and professional groups of people.
  • Plan, coordinate, and facilitate services for clients of the Dragonfly program.
  • Ability to operate within Electronic Medical Records (EMR) system.
  • Ability to operate tele-health platforms.
  • Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint, Office 365, Teams, Zoom and other communication/meeting platforms.
  • Possess a valid driver’s license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.
  • Ability to pass a criminal background check.
  • Ability to pass a pre-employment drug screen and TB test.

Responsibilities

  • Will provide mental health services to children, youth and care providers in settings such as client residence, school, community locations, tele-health and clinic sites.
  • Will provide mental health services aligned with Consumer Treatment Plan.
  • Will provide mental health services aligned with the CFT.
  • Will provide mental health services as a participant or coordinator of the ICC and IHBS service modalities.
  • Will provide mental health interventions that focus on behavioral improvements such as teaching social skills, coping skills, anger managements skills to help clients achieve treatment plan goals.
  • Will provide case management services intended to reduce barriers to treatment and provision of needed services through linkage to other community partners.
  • Will collaborate with community partners as part of mental health activities.
  • Will complete progress notes documenting service provision in electronic medical record (EMR) in timely manner and in accordance to standards designated by Program Director.
  • Employees shall follow the reporting requirements for incidents of Child Abuse, Elder Abuse, Adult Abuse, Dependent Adult Abuse and Tarasoff warnings per California reporting laws.
  • Will attend job-related trainings and also participate in a variety of in-service training, and seminars.
  • Will attend weekly staff meetings.
  • Physical presence at the program is required.
