About The Position

The Team Nurse for Adult Clinical Community Services Team at ServiceNet plays a crucial role in providing medical, psychiatric, health, and wellness consultation to the ACCS teams, as well as delivering direct services to select individuals in our outreach community and GLE programs. This full-time position operates Monday through Friday from 9 AM to 5 PM, with a starting salary of $72,800. The Team Nurse will be assigned a clinical caseload and will be responsible for writing and implementing comprehensive psychiatric assessments and treatment plans. A respectful, person-centered approach and effective care coordination within a multi-disciplinary team model are essential for success in this role. In this position, the Team Nurse will provide education, training, and consultation to staff on health-related issues, medication administration, and other medical/healthcare topics. They will oversee medical interventions and coordinate related services for individuals assessed to be in need of medical support through their formal treatment plan or updated team evaluation, both at residential sites and in community settings. The Team Nurse will work one-on-one with identified individuals to help them achieve their health and wellness goals, providing medication education, administration, and oversight to medically complex individuals as needed, ultimately aiming for medication independence. Additionally, the Team Nurse will act as a liaison to internal and community health providers and care coordination entities to facilitate treatment and ensure positive outcomes for individuals across all levels of care. Participation in various meetings, including intakes, staff meetings, external treatment meetings, care coordination meetings, and meetings with DMH and Crisis teams, is also required. The Team Nurse will document all interactions, communications, and assessments within required timeframes and provide coverage for the Clinical Nurse Director as needed, participating in the nursing on-call rotation as assigned. Other duties may be assigned by the Clinical Nurse Director, Contract Director, or Clinical Team Leader.
