About The Position

We are seeking a community-based Mental Health Professional to join our team in our Bethlehem, PA location. Position Responsibilities: Integrated Client Advocacy & Wellness: Advocates for an assigned caseload by supporting integrated physical and behavioral health needs through a holistic, person‑centered approach and collaboration with medical and behavioral health providers. Care Coordination & Community Linkage: Coordinates care during medical and psychiatric hospitalizations, manages aftercare and follow‑up, and connects clients to outpatient services and community‑based resources. Clinical Interventions & Goal‑Oriented Planning: Delivers evidence‑based clinical interventions and develops individualized, time‑limited service plans focused on increasing client independence. Assessment, Education & Health Management: Conducts routine assessments to monitor outcomes while educating clients and natural supports on diagnoses, treatments, insurance, lab work, and medication compliance. Community‑Based Service Delivery: Provides services in homes, community settings, and appointments, utilizing a personal vehicle to meet with clients daily (mileage reimbursed). Other duties as assigned.

Requirements

  • Master's degree in social work, psychology, counseling or related field of study is required.
  • Valid driver's license and vehicle required.
  • This position requires travel to various schools, homes, and other community locations to provide services and support clients.

  • Experience in mental health field and co-occurring/substance use is preferred.
