About The Position

The Mental Health Professional in Family-Based Mental Health Services (FBMHS) is the lead member of a multidisciplinary team and is responsible for treatment plan development. The Mental Health Professional serves as team leader providing team and individually delivered therapy and case management to children and their families in the home and/or community. Therapists work with the child and family to improve overall family attachments and cohesion, self-regulation, and anchor a support system. This position is required to work hours that coincide with client schedules, including day, evening, and/or weekend hours.

Requirements

  • Education -. Graduate Degree in psychiatry, psychology, social work, nursing, education, rehabilitation, or any graduate degree in the field of human services; OR Registered Nurse with five years’ experience in the mental health field, plus have certification by the Department of Public Welfare, Office of Mental Health (OMH), as a mental health family-based worker; OR Bachelor’s Degree in psychology, sociology, social work, nursing, rehabilitation, education, pre-med, theology, anthropology, or any degree in the field of human services, plus certification by the Pennsylvania Certification Board as a mental health family-based worker.
  • Experience Minimum of 2 years’ experience in a Children/Adolescent Social Service Program (CASSP)
  • Valid driver’s license

Responsibilities

  • Develops and maintains positive relationships with key stakeholders.

  • Develop meaningful relationships and collaborative with external resources for community linkage and aftercare planning.
  • Directs Communication with Management and Executives.
  • Establishes and maintains frequent and regular communication with Program Director, Program Coordinator and Operations Director as needed.
  • Report treatment difficulties to supervisor.
  • Identifies and executes on growth opportunities.
  • Ability to acknowledge and establish areas of personal and professional growth in annual evaluations.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess strong interpersonal skills to establish and maintain effective working relationships with client, child, parent/guardian, treatment team members, and other personnel involved in treatment.
  • Must possess ability to establish and maintain effective working relations with children, families, and staff from a variety of cultures and social backgrounds.
  • Must have knowledge of various therapy techniques and counseling strategies applicable to serving complex family systems.
  • Must have knowledge of child development and common behavioral disorders exhibited by children/adolescents and adults with persistent mental health challenges.
  • Must have strong written and verbal communication skills.

  • Sets meaningful, measurable, and attainable treatment plan goals and objectives for monthly treatment planning as well as professional development.
  • Develop individualized treatment plans through collaboration with the family and Mental Health Worker.
  • Allocates time and attention based on what is most important to achieve key goals and objectives.
  • Effectively organizes and balances tasks and priorities to keep multiple projects on track.
  • Maintains client schedules to assure we are meeting their needs and scheduling when they are available.
  • Submitting electronic health records and time worked within expected time frames.
  • Attend mandated, group, individual and team supervision as scheduled.
  • Maintain an organized schedule to include all methods of service delivery (team, individual, group, etc.) service delivery in accordance with regulations.
  • Adhere to scheduled sessions and deliver appropriate authorized units of service prescribed.
  • Has an authentic interpersonal style that engages others and encourages high performance in: Attend trainings as directed by supervisor, in accordance with company policy, and pursuant to applicable regulations.
  • Attend required supervision sessions and staff meetings in accordance with program/company requirements.
  • Participated in 3-year Eco-systemic Structural Family Therapy training program through Philadelphia Child and Family Training Center (family based only)

  • Communicates with other providers and coordinates care regularly with all individuals involved in treatment.
  • Adheres to HIPAA privacy regulations and Department of Human Services/CASSP principal guidelines.
  • Adheres to Mandated Reporter protocol and effectively communicates concerns with program management.
  • Work collaboratively with members of the treatment team and other service providers to ensure comprehensive case management.
  • Uses creativity appropriately to drive progress and encourages others to do the same.

  • Proactively plans for all session to promote an intentionally postured service approach.
  • Independently research creative and relevant interactive and evidenced based therapeutic interventions.
  • Uses healthy and effective methods to manage stress (e.g., maintaining appropriate diet, exercise, and sleep schedules); reacts effectively when faced with high-pressure situations.
  • Provides timely and professional service to both internal requests and the requests of the people we support and customers; is responsive to the people we support and customers' needs and requests.
  • Is always courteous to the people we support or customer and considers the needs of the people we support or customer when making decisions.
  • Being available for 24/7 crisis support in coordination with partner/team.
  • Being available to transport client and/or families in the community, as necessary.
  • Be available to de-escalate/divert hospitalization and/or assist in facilitating a hospitalization if deemed medically necessary.
  • Understands and follows company policies; encourages others to follow work rules; calls attention to actions that may violate policies and procedures.
  • Complies with basic job requirements and tasks; arrives at work on time as scheduled; proactively informs others when he/she encounters problems that may limit his/her ability to meet expectations.
  • Provide team (60%) and individually (40%) delivered therapy to children and their families in the home and/or the community.
  • Serve as lead clinician and provide clinical guidance to Mental Health Worker.
  • Utilize safe crisis prevention techniques as deemed appropriate.
  • Document treatment data/results that are clinically relevant on progress notes and submit in a timely manner (within 24 hours of each encounter).
  • Complete client intake paperwork/assessments per company/program guidelines
  • Complete recurrent assessments within the regulated timeframes
  • Assist/participate in client psychiatric evaluations and medication checks, understanding symptomology and managing symptoms.
  • training new staff and acting as a lead therapist.
  • Uses personal influence to encourage and support mental health workers on their caseloads.
  • Participate in interdisciplinary team meetings and other miscellaneous meetings, as necessary.
  • Collaborate with other providers to secure appropriate aftercare/discharge services and community linkages supports during the course of treatment (county, housing, MH/MR, and all other providers involved)
  • Develop, implement, evaluate, and revise treatment goals and plans in collaboration with client and team members in accordance with regulated timeframes.
  • Coordinate detoxification services with external inpatient service providers and establish linkage to self-help programs as needed.
  • Assist clients in budgeting and daily living skills to maintain stability in home and community settings.
  • Transport clients when necessary
  • Complete criminal background Name checked in the registries. (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities)
  • Drug testing
  • Education verification and other credentialing based on position requirements.
  • Proof of employment history or references (if required)
  • Proof of driver's license, driver's insurance, and vehicle, IF required for providing transportation for individuals.
