As a Mental Health Professional, you will be responsible for maintaining accurate and timely case records per licensing and Medicaid standards, ensuring continuity and quality of care. You will provide direct support during crisis situations, helping clients to navigate and stabilize through effective case management. Additionally, you will coordinate and develop Individualized Service Plans with input from team members and stakeholders to align treatment goals. Your role will also involve delivering person-centered, trauma-informed, and psycho-educational interventions designed to support treatment objectives. You will facilitate individual, family, and group counseling sessions, utilizing your approved credentialing and skill set. Maintaining regular contact with case professionals, such as school personnel, social services, and juvenile justice representatives, is essential to ensure cohesive client care. You will also provide case updates and presentations in formal settings such as FAPT meetings, IEP sessions, and court appearances.
