About The Position
The Mental Health Practitioner Intake and Referral assesses patient’s needs for acute crisis treatment, determining the appropriate level of care and referring the patient accordingly. This position also provides counseling services including assessment, diagnosis, and treatment for patients experiencing social/behavioral/mental health issues. The role involves coordinating screening, admission, assessment, and referral of incoming and outgoing patients for psychiatric inpatient beds requests, making treatment recommendations, and conducting thorough crisis assessments and safety planning. The practitioner will also provide outpatient referrals for patients not meeting inpatient level of care and develop referrals to outside agencies and hospitals when necessary. The position requires maintaining knowledge of all state and federal regulations as they apply to the admission, care, and discharge of patients from an inpatient psychiatric facility.
