About The Position

This position facilitates the identification, screening, referral and case monitoring of juveniles in detention or the community who are identified as having mental illness in efforts to strengthen the linkages among courts, probation, detention, schools, health care, mental health, and other community-based services.

Requirements

  • Must have Master’s Degree in Social Work, Counseling, Psychology or similar field.
  • Must be flexible, independent and able to multi-task effectively.
  • Must hold a valid, unexpired Illinois driver's license.
  • This position requires extensive travel throughout the Kane County.

Nice To Haves

  • Preferred experience in working with youth with mental health issues and juvenile justice involvement.
  • Preferred Certified Alcohol and Drug Counseling (CADC) certificate.

Responsibilities

  • Responsible for screening youth involved in juvenile justice system or at risk to be involved for mental health disorders.
  • Responsible for maintaining an active caseload and providing services to clients and families.
  • Responsible for completing 15-1-hour appointments per week including but not limited to: therapy sessions, crisis shifts, and walk in hours.
  • Responsible for networking with collaterals on behalf of clients and the agency, including the provision of case management, client centered consultation, wraparound services and referral and linkage to community resources.
  • Responsible for attending community meetings on behalf of agency to promote program and maintain referral sources.
  • Responsible for completion of agency comprehensive assessment, individual treatment plan, progress notes, and Mental Health/Juvenile Justice Initiative screening and assessment tools for each client per FSA policy and the MHJJ Grant requirements.
  • Responsible for responding to emergency/crisis situations with active clients and emergency back-up for Crisis.
  • Responsible for discussing cases with supervisor in clinical supervision.
  • Responsible for reporting to supervisor on an on-going basis potentially problematic situations/cases, and emergency or crisis situations.
  • Responsible for completion of and meeting deadlines for all clinical paperwork for cases, including: Mental Health Assessments, IATPs, Psychiatric Referral Forms, CIS Billing Notes, Case Closing, and other clinical paperwork necessary or appropriate for maintaining case files.
  • Responsible for completion of and meeting deadlines for all MHJJ paperwork and reports.
  • Responsible for completion of and meeting deadlines for all agency administrative paperwork, including: 1009’s, CIS Billing Notes, timesheets, expense sheets, and any other administrative or agency requested reports.
  • Responsible for attending all agency meetings, 9 team meetings per year, clinical supervision, and any additional program meetings.
  • Responsible for participating in Utilization Reviews.
  • Responsible for serving on agency and/or program committees when requested by supervisor.
  • Responsible for maintaining clinical charts in compliance with Rule 132 and Rule 140.
  • Responsible for educating the community regarding the MHJJ program in the service area.
  • Responsible for facilitating positive networking relationships with community service providers and/or networks as assigned by supervisor.
  • Be available to help develop materials and participate in special community education projects/presentations as requested by administrative staff.
  • Pursue and maintain licensure and/or certification that are available and appropriate to educational/professional background.
  • Perform other duties as assigned by supervisor.
  • Perform incidental business related travel as needed.
  • Participate in agency Risk Management and Continuous Quality Improvement activities.

  • Participate in projects or other duties as assigned by supervisor.
