About The Position

NaphCare is seeking a dedicated Mental Health Discharge Planner to join our team at the Wayne County Jail in Detroit. This full-time position involves working closely with patients to facilitate their discharge planning, ensuring they have the necessary support and resources for a successful transition. The role requires collaboration with healthcare professionals and community service providers to create effective discharge plans and assist patients in navigating their post-release care.

Requirements

• Must hold and maintain a Bachelor's Degree in a human service field.
• A valid CPR card is required.
• Knowledge and experience in the development of mental health discharge plans and coordination with community service providers.

Nice To Haves

• Experience in correctional healthcare is preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Conduct rounds with physicians and mental health staff to discuss placement options with patients.
• Organize records for patient transition and provide advice regarding community assistance organizations and transportation options.
• Involve patients in the discharge planning process.
• Assist patients in identifying a support network and provide instructions for future care and follow-up.
• Help patients complete all clinical and financial documentation needed for social service organizations.
• Coordinate with healthcare and security staff regarding patients' release dates.
• Act as a liaison for all referrals for involuntary examinations or crisis stabilization.
• Serve as a liaison for all inpatient care management.
