About The Position

The Mental Health Crisis Case Manager position at the Behavioral Health Assessment and Referral Center involves conducting crisis clinical assessments for patients seeking treatment at Edward Hospital and Elmhurst Hospital. The role requires assessing patients' clinical needs, obtaining initial diagnostic impressions, and providing treatment referrals consistent with the patients' medical and psychiatric status. The position also involves consulting with multidisciplinary treatment team members, completing admission paperwork, and participating in care conferences to discuss key factors affecting patients' treatment responses. The role is full-time, requiring 36 hours per week, with shifts from 7:00 am to 7:30 pm, and includes on-call responsibilities and weekend work.

Requirements

  • Master’s Degree or higher in a behavioral health field.
  • Current State of Illinois Registered Nurse or Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Clinical Professional Counselor (LCPC) licensure.
  • Current State of Illinois driver’s license required.
  • CPI (Crisis Prevention Intervention) certification within 90 days of hire.
  • Minimum of two years of experience in crisis/emergency services or behavioral health setting preferred.

Responsibilities

  • Conduct crisis clinical assessments for patients seeking treatment.
  • Assess patients' clinical needs and obtain initial diagnostic impressions.
  • Review previous documentation prior to the assessment process.
  • Provide treatment referrals consistent with the patient's medical and psychiatric status.
  • Identify potential problems that may adversely affect the patient.
  • Consult with multidisciplinary treatment team members on assessment, treatment, and discharge planning.
  • Complete admission paperwork and act as a resource for hospital staff.
  • Participate in care conferences and emergency department huddles.
  • Facilitate bed management planning and coordination with inpatient psych units.
  • Educate patients and families regarding the effects of illness and disability.
  • Follow up on documentation with local and state agencies as a mandated reporter.
